Essex-Windsor EMS have launched the Vulnerable Patient Navigator Program.
Eight paramedics in the community will work in identifying all patients who fit a certain criteria that call EMS more than three times in six months.
The goal is to connect those people with resources that readily available to them and also reduce the number of unnecessary ambulance calls.
Six of the eight VPN paramedics were on hand for an event Friday to launch the program.
